Veterans
Do you need help obtaining your DD-214 discharge, military
service records, or medals that you earned while serving in the military? Are
you having a problem with the VA? Do you know a veteran who is homeless or in
danger of becoming homeless? Call Sen. Leahy's Burlington office at 1-800-642-3193 if you
need my help.
Records or Medals Requests
The veteran (or next of kin, if veteran is deceased) must
complete and sign a
DD Form 180 (Request Pertaining to Military Records) and a
Privacy Release. Mail these forms, along
with a copy of your DD-214 (if available) to: Sen. Patrick Leahy, 199 Main
Street, Burlington, VT 05401. Sen. Leahy will make an inquiry to the National Personnel Records Center
on your behalf.

Emergency Assistance
Friends of Veterans
is an all-volunteer, non-profit organization of veterans (and people who work with
veterans) who offer emergency support or service coordination for veterans of
good service who are in financial, social or physical distress. If you
know a veteran in danger of losing his/her home, contact FOV at
802-296-8368 or FOV@valley.net The
Vermont
Office of Veterans Affairs may
be another resource for emergency assistance and referrals. You can also speak
with the veterans caseworker in Sen. Leahy's Burlington office.
Vermont Vet Centers
The Vet Center program
was established by Congress in 1979 out of the recognition that a significant
number of Vietnam era veterans were still experiencing readjustment problems.
Vet Centers are community based and part of the U.S. Department of Veterans
Affairs. The goal of the Vet Center program is to provide a broad range of
counseling, outreach and referral services to eligible veterans in order to help
them make a satisfying post-war readjustment to civilian life. Vet Centers
